This role involves ensuring a smooth and well-organized transition for patients moving from inpatient care to follow-up treatments, whether stationary or outpatient. The Discharge Manager will be responsible for coordinating these transitions, advising patients on eligibility and cost coverage, and handling all administrative tasks with insurance providers, rehabilitation clinics, and other follow-up facilities. Key to this position are strong organizational skills, empathy, and effective communication, as the manager acts as a crucial link between patients, the clinic, and external institutions.
